  • 1) Via Mail - post completed form(s) AND payment early enough that is will reach the following address by January 30, 2010:

  • 2) Via sealed envelope - completed form(s) AND payment maybe dropped off in a sealed envelope by January 30, 2010 before 5:30PM at:

    Cobb Swanson Music assumes NO responsibility, except to hold the forms for the RMFA.

  • 3) In person - if you have questions, receive help from RMFA volunteers on January 30, 2010 between 1:00pm and 5:30pm at:
